Dandruff on the eye (blepharitis)

Ocular blepharitis (dandruff on the eye) is an inflammation of the eyelid margin. It may appear alone or be combined with staphylococcal blepharitis or Meibomian gland dysfunction. The inflammation is located on the anterior margin of the eyelid, and may form oily or greasy crusts on the eyelids, eyelashes, eyebrows, and scalp.

Patients with seborrheic ocular blepharitis usually have increased Meibomian gland secretions, which become opaque. These secretions can cause the sensation of dandruff on the eye, a common discomfort.

Symptoms

The symptoms include chronic redness of the eyelids, a burning sensation, and sometimes a foreign body sensation. A small percentage of patients develop keratitis or conjunctivitis. Keratitis is characterized by punctate epithelial erosions distributed over the lower third of the cornea. Approximately one third of seborrheic blepharitis cases present with reduced tear production.

Treatments

Eyelid hygiene is the main treatment for patients with blepharitis. If inflammation is a prominent component of the eye dandruff, a short course of topical steroids applied to the eyelid margin can be useful. If the blepharitis primarily affects the posterior margin of the eyelid, systemic antibiotics are the cornerstone of treatment. Blepharitis caused by bacteria responds to topical antibiotics.
