Recurrent Corneal Erosion

Erosión corneal recurrente

A recurrent corneal erosion usually appears in eyes that have suffered a sudden, erosive, or puncturing injury (nails, paper cut, tree branch, etc.). They can also occur in eyes with pre-existing epithelial basement membrane dystrophy. The superficial injury produces an epithelial abrasion that heals quickly, without leaving clinical signs of the damage.

After a variable interval, from days to years, the symptoms suddenly reappear. This recurrence is usually not accompanied by an evident episode to indicate it.

Symptoms

They are characterized by sudden onset eye pain, usually at night or upon waking, combined with redness, photophobia, and tearing. Each episode has variable intensity and duration.

Treatments

Conservative treatment in the acute phase of corneal erosions consists of frequently lubricating the affected area with antibiotic ointments and cycloplegia. Afterwards, preservative-free lubricants or hypertonic saline solution are used during the day, and in the form of ointment at bedtime, for 6–12 months, to promote correct epithelial adhesion.

In certain cases, therapeutic contact lenses are useful, although it is important to establish proper follow-up.

When conservative treatment cannot control the symptoms, a more suitable surgical treatment is indicated. Phototherapeutic keratectomy (PTK) with excimer laser is an alternative treatment for patients with recurrent corneal erosions. By creating a large superficial ablation area, this procedure minimizes refractive effects, and is also used to simultaneously correct a myopic refractive error.
